Social Service Designee
- Welcome new residents and provide psychosocial support and monitor their comfort, concerns, and needs.
- Assesses each resident's needs at admission and thereafter with input from the resident, resident's representative, and develops the social service care plan, under the director of the Social Worker.
- Helps residents utilize community resources through referrals when the services needed are not provided by the facility.
- Reviews care plans and writes progress notes of a resident's response to the Social Service care plan when required by facility policies.
- Monitors nursing reports, incident reports, and complaints to identify resident changes, concerns, or incidents.
- Coordinate discharge planning communications and documentation.
- Performs patient transportation duties as needed.
- Supports and abides by the mission, values, and policies of CHCS in all activities.
- On-going compliance with CHCS educational requirements.
- All other duties as required.
