The Position:
The experienced Social Worker with a clinical psychosocial background is an integral member of the RCCA Oncology Care Team that is responsible for ensuring safe, reliable and highest quality of care provided to the patient and their families. Responsible for determining nature of patients situation through interviews after receiving referrals or learning of results of Distress assessments, suicide ideations, depression screening test results, and ensuring medical, psychological, emotional, and social information are completed and documented in EMR. Establishes course of action by exploring options; setting goals with patient, their families and caregivers, especially around adherence with chemotherapy and other medications. Fosters patients action or adjustment by interpreting attitudes and patterns of behavior; explaining and pointing out new options such as participating in clinical trials or initiating advance care planning and when appropriate end of life conversations. Maintains record of encounters by documenting in EMR and engaging physician in patient social, cultural and economic situation. Assist the Care Team in completing 13-point Institute of Medicine care management plan. Maintains and upheld RCCA operations by following policies and procedures; participating in quality reviews and other continuous quality improvement activities; reporting needed changes and offering solutions to effectuate change. Complies with federal, state, and local legal requirements by studying existing and new legislation; enforcing adherence to requirements; advising management on needed actions.
Responsibilities and Duties:
- Appraising patients circumstances, needs, predicaments, and inner strength in order to establish explicit objectives
- Establish/create procedures to help patients and their families enhance their total well-being and cope with their circumstances (receiving cancer treatments)
- Give assistance to patients in the area of adjusting to new changes and challenges like having to stop working, losing
benefits, inability to make appointments, and other disruptions in what their life used to be before contracting cancer - Obtains assistance for patients and their families by referring him/her to community resources; arranging for
appointments; establishing rapport with other agencies. - Organize resources for the use of the patient and families and introduce them to the appropriate support groups and assist in
completion of Survivorship Plan - Monitor and evaluate clients progress and modify treatment plans accordingly
- Maintains client confidence and protects operations by keeping information confidential
Knowledge and Experience:
- Proven work experience as a social worker
- Working knowledge of social theories and practices
- Social perceptiveness and empathy
- Ability to build and maintain professional helping relationships
- Ability to relate and communicate with diverse population and groups
- Resilience along with ability to assess situations
Special Skills Requirements:
- Emotional maturity, unprejudiced and sensitivity to clients and their problems
- Significant ability to set margins and agree with the boundaries of what can be achieved within a particular time
- Effective listening skills and ability to ask related questions per time
- Ability to manage information verbally conveyed and which is essential to the counseling aspect of social work
- Be conscious of body language and social forms of cultural behavior
- Efficiency in organizing, managing and prioritizing the analytical aspects of social work in providing purposeful services to clients
- Ability to reach a compromise and work effectively with multiple parties
- Self-starter, able to start initiatives and work with minimal supervision
- Bilingual preferred
- Excellent oral and written communications skills
- Experience with EMR
Education:
- BS degree in social work (BSW), psychology or sociology
- Masters Degree in Social Work preferred
- 5+ years clinical social work experience
- Valid and current LCSW or LMSW license
Physical Demands:
- Sits constantly with occasional walking
- Frequent travel to headquarters and practices and divisions
