Under the direction of the Assistant Administrator for Quality Improvement, the CLINICAL QUALITY ANALYST will primarily assure abstraction needs are met for assigned programs, provide expertise for case reviews and/or quality and process improvement, leadership and data abstraction support for Quality Improvement and Patient Safety efforts related to internal, Regulatory and Public Reporting of Clinical Quality Improvement/Patient Safety measures. Review and track performance for selected in-patients and out-patients with the goal of achieving a variety of targeted quality and safety metrics. This position will identify opportunities to improve documentation and provision of care related to care processes and selected quality care indicators. In addition, this position will participate actively in departmental functions including analysis, special projects and presentation of information and reports.
This position will also:
Provide clinical and administrative expertise and chart/data abstraction support for internal and external quality metrics such as TJC Oryx Core Measures, QualityNet, CMS-National Voluntary Reporting, HAC program and Annual Payment Update, COAP, Leapfrog and other assigned or ad hoc quality improvement projects/programs.
Serve as a QI advocate/champion for quality improvement and patient safety projects related to advancing equity at HMC in partnership with the HMC Equity Council.
Maintain current knowledge of related internal/external data requirements and data definitions for all assigned data elements and serve as an institutional resource related to selected internal/external data requirements to help educate stakeholders.
Recognize and recommend appropriate and adequate documentation related to selected quality care indicators and performance measures.
Provide ongoing data validation, data quality and data correction support for registries and other quality improvement clinical data.
Assist in the development and implementation of methods for real-time case identification for support of operational activities related to selected populations.
Network and coordinate with other team members focused on documentation and implementation of care measures such as other QI staff, Clinical Documentation Specialists, IT/Analystics, Finance and quality work groups.
Assess compliance with performance standards for internal and external QI programs:
Provide education, references and identify resources to assist with implementation and documentation of care processes
View current systems and identify areas for operational change, assist in collaboratively implementing change and tracking outcomes of change
Assist in the development and distribution of reports related to data analysis
Support identification and analysis of selected case review:
Independently abstract and analyze clinical care from complex medical record system as requested Identify potential deficiencies in clinical care and refer cases for expert review as needed
Support morbitity and mortality case selection
Support intensive reviews of selected cases
Provide documentation as required by external agencies related to case reviews
Participate in providing house-wide support for quality improvement and patient safety initiatives.
Present data and recommendations periodically to quality improvement, clinical and management bodies such as the Quality Improvement Committee, Organizational Improvement Metrics, Patient Safety Committee, Cardiology Quality Improvement Committee, and others as requested
Serve on committees as directed such as the Quality Improvement Committee, Equity Workgroups and others related to quality improvement, as indicated, providing updates on selected data/case reviews to committees as requested
Participates on subcommittees or special projects that contribute directly to the primary duties outlined for the position
Fulfill other duties as assigned.
REQUIRED:
BA or BS in Nursing or equivalent experience The Quality Improvement Clinical Quality Analyst must have 3-5 years of recent experience with abstraction of clinical data, quality improvement and patient safety activities (or equivalent clinical background) in an acute care setting. Demonstrated ability to coordinate information gathering and dissemination within a complex administrative system. Demonstrated ability to prepare and present information and communicate effectively with all levels of the hospital-wide quality improvement team and outside regulatory agencies. Demonstrated ability to assist in facilitating multidisciplinary work groups including clinical providers, quality improvement team members and other HMC/UWM system constituents from different departments and levels. Ability to work effectively and efficiently with several complex data collection and data registry collection systems. Ability to analyze data/care processes for identification of opportunities for improvement in care. Understanding of data validity, chart abstraction and coding issues within the electronic medical record and demonstrated ability to work effectively and efficiently with medical records. Highly self-directed and creative.
