Harborview Medical Center has an outstanding opportunity for a full-time, day shift, REHAB CARE MANAGER/PRE-ADMISSIONS COORDINATOR.
The Rehab Care Manager for the inpatient rehabilitation medicine unit (4WH-Trauma/Rehab Medicine) coordinates and facilitates care for patients who meet criteria for admission to receive comprehensive inpatient rehabilitation. This position leads the interdisciplinary care team which includes nursing, physicians, rehabilitation therapies, rehabilitation psychology and social work staff regarding a wide range of actions in order to maximize the clinical and financial outcomes for all rehabilitation inpatients that will result in smooth transitions from acute care to inpatient rehabilitation and then to outpatient services as needed. The care manager is the key leader of the health care team in working with the team, patients and family to identify needs and to develop mutually agreed upon plans and goals.
This position works closely with the Trauma Rehabilitation Admission Coordinator, Rehabilitation Patient Care Coordinator, and Care Managers in the Rehabilitation clinic to ensure continuity of care and efficient use of resources by focusing on patient care goals. The Care Manager makes independent judgments and decisions within the scope of practice and takes actions regarding a wide range of routine and complex patient care issues. The Care Manager performs specific functions related to clinical practice, education, and/or management.
The individual in this role will demonstrate knowledge and understanding of patient privacy rights. Maintain confidentiality of all medical, financial, or other sensitive materials and information in printed, electronic or verbal form, which may jeopardize the privacy of patients. Access and use the minimum necessary patient identifiable information to perform job responsibilities and duties and only for authorized purposes.

REQUIREMENTS:
A Bachelor’s degree from a recognized school of physical therapy or completion of a Physical Therapy program AND Current Washington State Physical Therapist license.
OR
Graduation from an accredited Occupational Therapy Program AND successful completion of American Occupational Therapy Association certification examination AND Current Washington State Occupational Therapist license.
OR
A Master’s degree in Speech-Language Pathology/Audiology or Communicative Disorders AND A Certificate of Clinical Competence from the American Speech-Language and Hearing Association AND Current Washington State Speech/Language Pathology license.
OR
A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND Current Washington State Advance Social Worker license OR Independent Clinical Social Worker license.
AND
Minimum of two years of professional Inpatient Rehabilitation experience, in one of the disciplines listed above.
